As a Tacoma Endoscopy Center patient, you are responsible for:
· Following Directions: The patient is responsible for following direction in
procedure preparation and follow-up care.
· Asking Questions: The patient is encouraged to ask questions of the physician
or staff if he/she is unsure of any aspect of the procedure or follow-up.
· Providing Accurate Information: The patient is asked to provide clear and
accurate information (to his/her knowledge) regarding name, address, employer,
age, social security number, insurance coverage, medical history and allergies.
· Accepting Financial Responsibility: The patient is responsible for any portion
of the bill not covered by his/her insurance plan.
