What Every Patient Needs To Know

· If you are a new patient to this practice, we ask that you arrive 30 minutes prior to your scheduled appointment. For established patients, please arrive 10 minutes prior to your appointment. This allows time for registration and to be admitted to an exam room so your health care provider may see you at your appointed time.

· So that quality time may be spent with each of our patients, preference is given to patients who arrive on time for their appointments. Patients unable to arrive on time will need to be re-scheduled on another day.

· So that your health care provider may devote their full attention to you during your scheduled appointment, we expect that other arrangements for childcare will be made for your children. Children may not be left unattended in the waiting room.

· We ask that patients call the office if they have problems or questions related only to the medical condition for which they were referred. Other healthcare concerns need to be directed to your primary care physician. The nursing staff listens to their voicemail messages frequently throughout the day and make every attempt to return calls in a timely manner. When your healthcare provider is in the office, return calls may be delayed while full attention is given to patients in the office. We recognize that all patient calls are important. However, due to the high volume of calls and or the nature of the calls, there may be a delay in the nurse getting back to the patient.

· You will be notified of test results and any recommendations for follow-up care after your health care provider has reviewed the results. If you have an appointment scheduled, a card will be mailed to you explaining the same. If you have not received your test results in two weeks, please feel free to call your health care provider's nurse for the results.

· For prescription refills, please call your pharmacy. They will contact our office, if necessary. You must allow two working days for all prescription refills. If your prescription requires prior authorization from your insurance company, please allow more time. By law, you must be seen at least annually in order to continue to receive prescriptions or refills.

· Patients who fail to show for scheduled appointments will be charged $50 if their appointment was for an office visit; or $100 if they missed an appointment for a procedure. Patients may re-schedule with no penalty charge if they call at least 48 hours prior to their scheduled appointment.

· Because of the amount of time involved with completing various forms (such as disability, FMLA, life insurance, etc.), there is a charge for completion of these forms. Due to the various requirements, it is necessary to charge $100 for each 15 minutes needed for completion. You may find it less expensive to contact your primary care physician first for completion of these forms.
